Centrum fizjoterapii Physiomedica to miejsce, w kt贸rym indywidualnie i profesjonalnie podchodzimy do ka偶dego pacjenta. Staramy si臋 bardzo dok艂adnie diagnozowa膰 i analizowa膰 ka偶dy przypadek, dopasowywa膰 metody terapeutyczne oraz prowadzi膰 spersonalizowan膮 terapi臋 Pacjenta.

Nasi specjali艣ci to wysokiej klasy do艣wiadczeni fizjoterapeuci. Prowadzimy indywidualne zaj臋cia z zakresu terapii manualnej, terapii wad postawy, masa偶u i fizjoterapii uroginekologicznej. Oferujemy kinesiologtaping, ig艂oterapi臋, mo偶liwo艣膰 wykonania USG w trakcie wizyty oraz dobieramy indywidualne wk艂adki ortopedyczne.

REGULAMIN

Regulamin Centrum fizjoterapii Physiomedica

  1. Um贸wion膮 wizyt臋 nale偶y odwo艂a膰 min. 24 h przed jej terminem. W innym przypadku zostanie pobrane 50% jej warto艣ci.
  2. Dopuszczamy jednorazow膮 sytuacj臋 wyj膮tkow膮, gdzie op艂ata nie zostanie pobrana od pacjenta (dotyczy to TYLKO pacjent贸w, kt贸rzy odbyli co najmniej jedn膮 wizyt臋 w naszej klinice).
  3. Ch臋膰 uzyskania faktury nale偶y zg艂osi膰 przed pierwsz膮 wizyt膮. W przypadku kilku wizyt wystawiamy faktur臋 zbiorcz膮.
  4. Godziny wizyt um贸wionych przez portale mog膮 ulec zmianie do 15 minut. W przypadku samodzielnego prze艂o偶enia wizyty przez pacjenta za po艣rednictwem portali rejestracyjnych zastrzegamy sobie prawo do lekkiej korekty nowego terminu wizyty, po uprzednim poinformowaniu pacjenta.
  5. Istnieje mo偶liwo艣膰 wykupienia pakietu 5 wizyt, ale najpierw nale偶y odby膰 pierwsz膮 wizyt臋 (konsultacj臋 fizjoterapeutyczn膮) w plac贸wce..
  6. Pakiet 5 wizyt jest wa偶ny przez p贸艂 roku od daty zakupu.
  7. W przypadku rezerwacji wizyty dokonywanej osobi艣cie w recepcji przez nowe osoby bez wcze艣niejszego zapisu, na termin przypadaj膮cy w ci膮gu kilku dni od rejestracji, plac贸wka zastrzega sobie prawo do pobrania kaucji w wysoko艣ci 50% warto艣ci wizyty. Kaucja zostaje zaliczona na poczet realizowanej us艂ug.

Um贸wienie wizyty jest r贸wnoznaczne z akceptacj膮 regulaminu plac贸wki.

Clinic Policies:

  1. Appointments must be canceled at least 24 hours in advance. If not, 50% of the appointment fee will be charged.
  2. A one-time exception may be granted, in which the fee will not be charged. This applies only to patients who have previously attended at least one appointment at our clinic.
  3. Requests for an invoice must be made before the first appointment. For multiple appointments, a single consolidated invoice will be issued.
  4. Appointment times scheduled through online portals may be adjusted by up to 15 minutes. If a patient reschedules an appointment independently via an online portal, we reserve the right to slightly adjust the new appointment time, with prior notice to the patient.
  5. A package of 5 appointments can be purchased, but the first visit (physiotherapy consultation) must be completed at the clinic.
  6. The 5-visit package is valid for six months from the date of purchase.
  7. In the case of appointments booked in person at the reception desk by new clients without prior registration, for dates scheduled within a few days of booking, the clinic reserves the right to collect a deposit amounting to 50% of the appointment value. The deposit will be credited toward the service provided
